Custodial Care Services
Bicol Sanitarium, as a healthcare facility initiates advocacy efforts in order to reduce stigma and discrimination against people and families affected by the disease. It empowers people by allowing them to participate more fully in the life of the community and be able to meet their own needs,gain social acceptance to their disability and live a dignified life. Admitted patients afflicted by leprosy are categorized into three and are based on the servicesthat they are receiving namely Clinical,Palliative and Custodial Care.Specifically, Custodial Care caters to those patients who have completed Multidrug-Therapy and have no clinical signs of the disease and co-morbidity. Some patients have difficulty returning home and decided to live inside hospital premises with their families. They are also enrolled in special programs and activities which aim to enhance their interaction with the people and become productive members of the community. These patients are provided with monthly stipend and weekly rations to continuously support their daily needs. Their activities are being managed by administrative service in partnership with St. Paul of Chartres Sisters guided by the hospital management and Custodial Care Committee.
The ulcers commonly found in the feet of the people affected by leprosy. If there is no nerve function, the pain cannot be felt by the patients that may results to further damage. Wound debridement, this is a usual procedure performed in rehab section. Another wound care is with the use of Unna Paste and Unna boots. Unna patch is usually made of Zinc Oxide paste applied uniformly to the entire gauze. The Zinc Oxide paste in the Unna patch maintains moist and promote healing within the wounds sites, this also contains gelatine,glycerine oil and distilled water. It is used to treat edema, ulcers and sores. The healing process of wounds needs proper footwear. Bicol Sanitarium established our own footwear section in order to help the patients with their healing process. This is important to the patients because without proper footwear many patients with wound will fail to fast recovery. Many patients with sensory loss may be at risk of developing wound when they walk long distance. The regular use of appropriate foot wear is on of the most important measures to prevent disability. This ensures proper foot care adequate attention to patient's wound and plantar ulcers to avoid further damage and deformities.
Nerve Function Assessment
It is a neurological examination clinical assessment of sensory and motor components of central and peripheral nervous system. This can be done with the use of a filament that can be point in a certain part of our extremities and will determine if there is numbness/paresthesia.Voluntary muscle testing will also determine if there is weakness at some point of the extremities. The result of nerve function assessment will be grade 0 if there is no deformity, grade 1 if there is numbness but no visible deformity and lastly is grade 2 if there is clawing and wound.
A variety of equipment used to aid patients in performing tasks or movement. Assistive device available in Bicol Sanitarium are crutches, quadcane and wheelchair. The therapist will fit and instruct the patient about how to care with this assistive device. The goal of this device is to achive independence and safety.
B. Monthly Socialization
Bicol Sanitarium conducts monthly socialization for admitted post- leprosy patients of the hospital. The patients participate in healthy lifestyle activities and different facilitated games for them to have fun. This is to build harmonious relationship among patients and staff of the hospital. It is also a tradition that gifts will be given to monthly birthday celebrators. Every month, there is an assigned program coordinator to give lecture about the monthly celebration of Department of Health Program. This will increase their awareness regarding the current health status. Aside from that, there is a quarterly module-based facilitation with topics depending on the current needs of the patients. This is done to address the present psychological needs of the patients in order for them to have positive mental health and growth.
C. Quarterly Urinalysis and Monthly Weight Monitoring
This is a standard operating procedure conducted monthly for weight monitoring and quarterly for urinalysis of patients to monitor their health status and for diagnostic purposes.Upon registration of attendance for the monthly socialization, each patient go to weighing scale to be recorded by the nurse assisting them. On the other hand, the quarterly urinalysis is done with assigned schedule made by the nurse in-charge for their laboratory requests.
